Currently, our youth group is meeting approx. once per month for special events.
For special events, a permission slip is required for any youth under the age of 18, unless a parent or guardian both provides transportation and attends the event.

Please download, print, fillout, and sign the permission slip and give the hardcopy to the person in charge before the start of the event.
